Indications

  • partial and secondary generalized tonic-clonic seizures
  • Primary generalized tonic-clonic seizures
  • Trigeminal neuralgia
  • Prophylaxis of bipolar disorder

Pharmacology

  • Depresses activity in the nucleus ventralis of the thalamus
  • Reduces synaptic propagation of excitatory impulses
  • Decreases summation of temporal stimulation
  • Limits influx of Na ions across cell membrane
  • Stimulates release of antidiuretic hormone (ADH)
  • Potentiates action in promoting reabsorption of water

Dosage & Administration

  • Available in tablets, controlled release tablets, and suspension forms
  • Dosage varies for different age groups (adults, children over 12, 12-15 years, 6-12 years, under 6 years)
  • Combination therapy with other anticonvulsants
  • For trigeminal neuralgia, dosage adjustment may be needed

Interaction

  • Increased metabolism with hepatic enzyme inducers
  • Possible interaction with oral contraceptives

Contraindications

  • Allergy to any ingredients
  • Problems with electrical message pathways in the heart
  • History of decreased blood cell production by the bone marrow
  • Hereditary blood disorders called porphyrias
  • Allergy to tricyclic antidepressants
  • Recent use of monoamine-oxidase inhibitor antidepressant (MAOI)
