Medicine Details

Indications

  • Partial and secondary generalized tonic-clonic seizures
  • Primary generalized tonic-clonic seizures
  • Trigeminal neuralgia
  • Prophylaxis of bipolar disorder

Pharmacology

  • Depresses activity in the nucleus ventralis of the thalamus
  • Reduces synaptic propagation of excitatory impulses
  • Stimulates the release of antidiuretic hormone (ADH)
  • Potentiates action in promoting reabsorption of water

Dosage & Administration

  • Initial dosage 200 mg b.i.d. for adults and children over 12 years
  • Maximum dosage 1600 mg daily in adults
  • Maintenance dosage 800-1200 mg daily for adults
  • Initial dosage 100 mg b.i.d. for children 6-12 years
  • Maximum dosage 1000 mg daily for children 6-12 years
  • Maintenance dosage 400-800 mg daily for children 6-12 years
  • Initial dosage 10-20 mg/kg/day b.i.d.or t.i.d. for children under 6 years
  • Combination therapy with other anticonvulsants
  • Trigeminal Neuralgia: Initial dosage 100 mg b.i.d.
  • Trigeminal Neuralgia: Maximum dosage 1200 mg daily
  • Trigeminal Neuralgia: Maintenance dosage 400-800 mg daily

Interaction

  • Galactorrhoea reported in women on oral contraceptives within the first two months of treatment
  • Interaction with hepatic enzyme inducers like Phenytoin
  • Possible increase in dosage required

Contraindications

  • Allergy to any ingredients
  • Problems with the electrical message pathways in the heart
  • History of bone marrow depression
  • Hereditary blood disorders called porphyrias
  • Allergy to tricyclic antidepressants
  • Use of MAOI antidepressants in the last 14 days

Side Effects

  • Common: dizziness, drowsiness, ataxia, dry mouth, abdominal pain, nausea, vomiting, leucopenia, proteinuria, bradycardia
  • Severe: Erythematous skin rash, aplastic anemia, hypotension, heart failure
  • Increased sun sensitivity
  • Adverse reactions in the hemopoietic, skin, and cardiovascular system

Pregnancy & Lactation

  • Pregnancy category D
  • Transfer to breast milk
  • Consideration for nursing mothers

Precautions & Warnings

  • May cause dizziness and drowsiness
  • Potential skin reactions
  • Rarely cause liver problems
  • Decreases blood levels of hormonal contraceptives
  • Recommendations for women requiring contraception
  • Caution in mixed seizures, elderly people, history of heart/kidney/liver disease, raised eye pressure, blood disorders
  • Caution regarding sudden discontinuation
