Indications

  • Treatment of panic disorder with or without agoraphobia
  • Treatment of Lennox-Gastaut Syndrome (petit mal variant)
  • Adjunct in the treatment of akinetic and myoclonic seizures
  • Treatment of absence seizures (petit mal) in patients who have failed to respond to succinimides

Pharmacology

  • Anticonvulsive effects
  • Sedative effects
  • Muscle relaxing effects
  • Anxiolytic effects
  • Enhancement of GABAergic neurotransmission
  • Suppression of paroxysmal activity

Dosage & Administration

  • Initial dose for adults with seizure disorders: not to exceed 1.5 mg/day divided into three doses
  • Initial dose for adults with panic disorder: 0.25 mg given in two divided doses
  • Pediatric dose for infants and children: 0.01-0.03 mg/kg/day divided into two or three doses
  • Maximum recommended daily dose: 20 mg

Interaction

  • No alteration of the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital observed
  • Effect on the metabolism of other drugs has not been investigated

Contraindications

  • History of hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Acute narrow angle glaucoma

  • May increase the incidence of generalized tonic-clonic seizures in certain patients

Use in Special Populations

  • Caution required in pediatric use due to increased salivation and bronchial secretion
  • Benzodiazepine pharmacologic effects appear to be greater in elderly patients
  • Renal impairment does not affect the pharmacokinetics

Overdose Effects

  • Symptoms: drowsiness, ataxia, dysarthria, nystagmus
  • May lead to areflexia, apnoea, hypotension, cardiorespiratory depression, and coma
  • Increased frequency of seizures at supratherapeutic plasma concentrations
