Indications

  • Treatment of panic disorder with or without agoraphobia
  • Adjunct treatment of Lennox-Gastaut Syndrome, akinetic and myoclonic seizures
  • May be indicated in patients with absence seizures who have failed to respond to succinimides

Pharmacology

  • Exhibits anticonvulsive, sedative, muscle relaxing and anxiolytic effects
  • Enhances GABAergic neurotransmission at inhibitory synapses
  • Suppresses various types of paroxysmal activity in seizures

DosageAndAdministration

  • Oral and injection administration for adults and pediatric patients
  • Dose adjustment based on response and patient's age/weight

Interaction

  • May not alter the pharmacokinetics of specific drugs
  • Effect on the metabolism of other drugs has not been investigated

Contraindications

  • History of hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Contraindicated in acute narrow angle glaucoma

  • Aphonia, coma, tremor, vertigo, confusion, depression, amnesia, hallucinations

  • May cause congenital malformations
  • Drug should only be administered to pregnant women if benefits outweigh risks
  • Small amounts may pass into maternal milk, breastfeeding should be discontinued if compelling indication

  • May increase incidence or precipitate the onset of generalized tonic-clonic seizures
  • Concomitant use of valproic acid may produce absence status

  • Increased saliva and bronchial secretion in infants and small children
  • Greater pharmacologic effects in elderly patients
  • Renal impairment does not affect pharmacokinetics
  • Plasma protein binding significantly different in cirrhotic patients

OverdoseEffects

  • Common symptoms include drowsiness, dysarthria, nystagmus
  • May lead to areflexia, apnoea, hypotension, cardiorespiratory depression, coma
  • Increased frequency of seizures at supratherapeutic plasma concentrations
