Description

Indications

  • Treats panic disorder with or without agoraphobia
  • Treatment of Lennox-Gastaut Syndrome
  • Treatment of absence seizures (petit mal)

Pharmacology

  • Exhibits anticonvulsive, sedative, muscle relaxing and anxiolytic effects
  • Enhances GABAergic neurotransmission at inhibitory synapses
  • Suppresses paroxysmal activity including different types of seizures

Dosage & Administration

  • Initial dose for adults with seizure disorders should not exceed 1.5 mg/day
  • Initial dose for adults with panic disorder is 0.25 mg
  • Dosage may be increased in increments of 0.5 to 1 mg every 3 days

Interaction

  • Does not alter the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital
  • Effect on the metabolism of other drugs has not been investigated

Contraindications

  • History of hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Acute narrow angle glaucoma

Precautions & Warnings

  • May increase the incidence or precipitate the onset of generalized tonic-clonic seizures in patients with different types of seizure disorders
  • Concomitant use with valproic acid may produce absence status

Use in Special Populations

  • May cause increased production of saliva and bronchial secretion in infants and small children
  • Greater pharmacologic effects in elderly patients
  • Renal impairment does not affect the pharmacokinetics
  • Plasma protein binding in cirrhotic patients is significantly different from that in healthy subjects

Overdose Effects

  • Common symptoms include drowsiness, ataxia, dysarthria, and nystagmus
  • May lead to areflexia, apnoea, hypotension, cardiorespiratory depression, and coma

Therapeutic Class

  • Adjunct anti-epileptic drugs
  • Benzodiazepine hypnotics

Reconstitution

  • For slow intravenous injection, contents of the vial must be diluted with water for injection prior to administration
  • For intravenous infusion, Epnil can be diluted in various diluting media
  • Intramuscular route should be used only in exceptional cases

Dosage Options

  • Adults: Initial dose should not exceed 1.5 mg/day
  • Pediatric Patients: Initial dose should be between 0.01 and 0.03 mg/kg/day for infants and children
  • Infants and children: half of a vial (0.5 mg) by slow IV injection
