Description
Lonapam 2mg Tablet is indicated for the treatment of panic disorder, with or without agoraphobia, as well as for the treatment of Lennox-Gastaut Syndrome, akinetic and myoclonic seizures. It exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects and enhances GABAergic neurotransmission at inhibitory synapses. The recommended dosage varies for adults and pediatric patients and can be administered orally or through injection. Lonapam interacts with various other drugs and has contraindications, side effects, and precautions to be aware of. It should be stored in a dry place away from light and heat.
Indications
Treatment of panic disorder, with or without agoraphobia, and adjunct in the treatment of the Lennox-Gastaut Syndrome (petit mal variant), akinetic and myoclonic seizures

Dosage and administration
- Oral:
- Adults:
- Initial dose not to exceed 1.5 mg/day
- Dosage increment of 0.5 to 1 mg every 3 days
- Maximum recommended daily dose 20 mg
- Panic disorder: Initial dose of 0.25 mg for adults

- Infants and children: Half of a vial (0.5 mg)
- Adults: 1 vial (1 mg)
Interaction
Does not appear to alter the pharmacokinetics of specific drugs, but its effect on the metabolism of other drugs has not been investigated
Contraindications
- Hypersensitivity to benzodiazepines
- Significant liver disease
- Acute narrow angle glaucoma

Precautions and warnings
May increase the incidence or precipitate the onset of generalized tonic-clonic seizures when used in patients with several different types of seizure disorders coexisting. The concomitant use of valproic acid and Lonapam may produce absence status
Special populations
- Pediatric use: May cause increased production of saliva and bronchial secretion in infants and small children
- Geriatric use: Benzodiazepine pharmacologic effects appear to be greater in elderly patients
- Renal impairment: Does not affect the pharmacokinetics of Lonapam
- Hepatic impairment: Plasma protein binding in cirrhotic patients is significantly different from that in healthy subjects
Overdose effects
- Symptoms: Drowsiness, ataxia, dysarthria, nystagmus, areflexia, apnoea, hypotension, cardiorespiratory depression, coma
- Treatment:
- Monitor vital signs
- Institute supportive measures
- Prevent further absorption using appropriate methods
- Consider the use of flumazenil

Reconstitution
- Slow intravenous injection: Dilute with 1 ml of water for injection prior to administration
- Intravenous infusion: Can be diluted for infusion with various diluting media
