Indications

  • Treatment of panic disorder with or without agoraphobia
  • Adjunct in the treatment of Lennox-Gastaut Syndrome
  • Treatment of akinetic and myoclonic seizures

Pharmacology

  • Exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects
  • Enhances GABAergic neurotransmission
  • Suppresses different paroxysmal activities in seizures
  • Shows effect on serotonin in animal data

Dosage & Administration

  • Initial and maintenance doses for adults with seizure disorders
  • Initial and target doses for adults with panic disorder
  • Initial dose for pediatric patients
  • Dosage for infants and children
  • Dosage for injection in infants, children, and adults

Interaction

  • No alteration in the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital
  • Effect on the metabolism of other drugs not investigated

Contraindications

  • History of hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Not recommended in acute narrow angle glaucoma

Side Effects

  • CNS depression, drowsiness, ataxia, behavior problems, abnormal eye movements, aphonia, coma, tremor, vertigo, confusion, depression, amnesia, hallucinations, increased libido, insomnia, psychosis, palpitations

Pregnancy & Lactation

  • Potential to cause congenital malformations
  • Use only if benefits outweigh risks during pregnancy
  • Cautious administration during breastfeeding
  • Possibility of withdrawal symptoms in newborns

Precautions & Warnings

  • May increase incidence of generalized tonic-clonic seizures in certain patients
  • Concomitant use with valproic acid may produce absence status

Use in Special Populations

  • Increased production of saliva and bronchial secretion in infants and small children
  • Benzodiazepine effects appear to be greater in elderly patients
  • No dose adjustment required in renal impairment
  • Influence on plasma protein binding in cirrhotic patients

Overdose Effects

  • Common symptoms include drowsiness, ataxia, dysarthria, nystagmus
  • Potential effects on vital signs, cardiorespiratory and CNS
  • Use of flumazenil as a benzodiazepine antagonist under monitored conditions
