Medicine Details

Indications

  • Treatment of panic disorder with or without agoraphobia
  • Treatment of Lennox-Gastaut Syndrome
  • Treatment of akinetic and myoclonic seizures
  • Adjunct in the treatment of petit mal variant of the Lennox-Gastaut Syndrome
  • Treatment of absence seizures (petit mal)

Pharmacology

  • Exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects
  • Enhances GABAergic neurotransmission at inhibitory synapses
  • Suppresses various types of paroxysmal activity including seizures

Dosage & Administration

  • Initial adult dose for seizure disorders: 1.5 mg/day divided into three doses
  • Maximum recommended daily dose for seizures: 20 mg
  • Initial adult dose for panic disorder: 0.25 mg given in two divided doses
  • Maximum target dose for panic disorder: 1 mg/day

Interaction

  • Does not appear to alter the pharmacokinetics of certain drugs

Contraindications

  • Hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Acute narrow angle glaucoma
  • Not to be used in pregnancy or lactation

Pregnancy & Lactation

  • Potential teratogenic effects
  • Drug should only be administered if potential benefits outweigh the risk to the fetus
  • Active ingredient may pass into maternal milk

Precautions & Warnings

  • May increase the incidence of generalized tonic-clonic seizures
  • Concomitant use with valproic acid may produce absence status

Use in Special Populations

  • In infants and small children, may cause increased production of saliva and bronchial secretion
  • Benzodiazepine pharmacologic effects appear to be greater in elderly and renal impairment patients

Overdose Effects

  • Symptoms: drowsiness, ataxia, dysarthria, nystagmus
  • Treatment: monitor vital signs, institute supportive measures, prevent further absorption, consider use of flumazenil

Therapeutic Class

  • Adjunct anti-epileptic drugs
  • Benzodiazepine hypnotics

Reconstitution

  • Preparation for slow intravenous injection
  • Preparation for intravenous infusion
  • Intramuscular injection only in exceptional cases
