Indications

• Treatment of panic disorder with or without agoraphobia
• Treatment of Lennox-Gastaut Syndrome, akinetic and myoclonic seizures
• Adjunct in the treatment of absence seizures (petit mal)

Pharmacology

• Exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects
• Enhancement of GABAergic neurotransmission
• Effect on serotonin
• Suppression of various types of paroxysmal activity

DosageAdministration

• Initial dose for adults with seizure disorders should not exceed 1.5 mg/day
• Initial dose for adults with panic disorder is 0.25 mg
• Initial dose for pediatric patients up to 10 years of age is between 0.01 and 0.03 mg/kg/day
• Adult injection dosage: 1 vial (1 mg) by slow IV injection or by IV infusion
• Pediatric injection dosage: half of a vial (0.5 mg) by slow IV injection or by IV infusion

Interaction

• No apparent alteration of the pharmacokinetics of certain drugs
• Effect on the metabolism of other drugs not investigated

Contraindications

• History of hypersensitivity to benzodiazepines
• Clinical or biochemical evidence of significant liver disease
• Acute narrow angle glaucoma

SideEffects

• Frequently occurring CNS depression
• Drowsiness, ataxia, behavior problems, abnormal eye movements, coma, tremor, vertigo, confusion, amnesia, hallucinations, increased libido, insomnia, psychosis, palpitations

PregnancyLactation

• Potential for producing congenital malformations
• Use during pregnancy only if benefits outweigh risks
• Administration during labor can cause irregularities in the heartbeat of the unborn child and other complications
• Active ingredient may pass into maternal milk
• Discontinuation of breastfeeding if compelling indication for use

PrecautionsWarnings

• May increase incidence or precipitate the onset of generalized tonic-clonic seizures in certain patients
• Concomitant use with valproic acid may produce absence status
• Special attention required in pediatric and geriatric use

SpecialPopulations

• Increased production of saliva and bronchial secretion in infants and small children
• Greater pharmacologic effects in elderly patients
• No dose adjustment required in patients with renal impairment
• Potential alterations in pharmacokinetics in hepatic impairment

OverdoseEffects

• Symptoms include drowsiness, ataxia, dysarthria, nystagmus
• May lead to serious cardiorespiratory effects and coma
• Benzodiazepine respiratory depressant effects more serious in patients with respiratory disease

Reconstitution

• Dilution with water for IV administration to avoid vein irritation
• Dilution for IV infusion in specific ratio with compatible diluting media
