Indications
- Anticonvulsant
- Treatment of status epilepticus
- Prevention of seizures during neurosurgery
Pharmacology
- Increases efflux/inhibits influx of sodium ions
- Stabilizes neuronal membranes
- Decreases seizure activity
- Extends effective refractory period
- Suppresses ventricular pacemaker automaticity
Dosage & Administration
- Adult dosage: 200-400mg daily
- Children dosage: 5 mg/kg per day
- Divided dosage for both adults and children
- Maximum dosage: 600mg/day for adults, 300mg/day for children
Interaction
- Potentiation with sedative drugs or ethanol
- Enhanced toxic effects of paracetamol, lithium
- Increased risk of osteomalacia with acetazolamide
- Decreased serum levels/effects with acyclovir, antineoplastics, benzodiazepines, etc.
- Increased serum concentrations with allopurinol, capecitabine, cimetidine, etc.
- Metabolism and effect interactions with various drug categories
Contraindications
- Hypersensitivity to phenytoin and hydantoins

Precautions & Warnings
- Periodic blood checking in prolonged treatment
- Discontinue if skin rashes appear
- Reported hyperglycemia resulting from drug's inhibitory effects on insulin release
- Caution in pregnancy and nursing mothers
