Indications
- Treatment of status epilepticus
- Prevention of seizures during neurosurgery
Pharmacology
- Acts as an anticonvulsant by affecting sodium ion movement
- Acts as an antiarrhythmic by extending effective refractory period
Dosage & Administration
- Adult dosage: 200-600 mg/day in divided doses
- Initial children's dosage: 5 mg/kg/day in divided doses
- Recommended children's maintenance dosage: 4 to 8 mg/kg
Interactions
- Potentiated effects with sedative drugs or ethanol
- Enhanced toxic effects with paracetamol and lithium
- Increased risk of osteomalacia with acetazolamide
- Interactions with various drugs affecting serum levels/effects
Contraindications
- Contraindicated in patients hypersensitive to phenytoin and hydantoins
Side Effects
- Occasional development of skin rashes
- Side effects may disappear with lower doses
Precautions & Warnings
- Periodic blood checks advisable during prolonged use
- Discontinue if skin rashes appear
- Use with caution in pregnancy and nursing mothers
