Indications
- Neuropathic Pain
- Adjunctive therapy in partial seizure and secondary generalized seizure
Pharmacology
Gabapentin is an anti-convulsant. It is a structural analog of gamma-amino-butyric-acid (GABA). All pharmacological actions following administration of Gabapentin are due to the activity of parent compound. Gabapentin binds with the alpha-2-delta subunit of voltage gated L-type Calcium channel, and inhibits branched chain amino acid transferase & probably inhibits neurotransmitter release of excitatory amino acid.
Dosage
- Initial dose of 300 mg for Neuropathic Pain
- Titration up to 1800 mg daily divided into doses for pain relief
- 900 to 1800 mg/day for epilepsy in three divided doses for patients over 12 years of age
- Starting dose of 300 mg three times a day for epilepsy in patients over 12 years of age
- Starting dose of 10-15 mg/kg/day for pediatric patients age 3-12 year
- Effective dose reached by upward titration over a period of approximately three days for pediatric patients age 3-12 year
Administration
Oral administration, with or without food
Interaction
- Antacids may reduce bioavailability by up to 20%
- Cimetidine may alter renal excretion

Pregnancy & Lactation
Pregnancy category C. Use during pregnancy if potential benefits justify potential risk to the fetus. May be secreted through breast milk, use during lactation if potential benefits justify potential risk to the baby.
