Description
Phenyltriazine antiepileptic used to treat epilepsy and bipolar disorder by inhibiting voltage-sensitive sodium channels and stabilizing neuronal membranes.
Indications
- Adjunctive therapy in partial-onset seizures
- Adjunctive therapy in primary generalized tonic-clonic seizures
- Adjunctive therapy in generalized seizures of Lennox-Gastaut syndrome
- Monotherapy in patients aged 16 years and older with epilepsy
- Treatment of bipolar disorder in patients aged 18 years and older
Pharmacology
- Inhibits voltage-sensitive sodium channels
- Stabilizes neuronal membranes
- Modulates release of presynaptic excitatory neurotransmitters
- Inhibits sodium currents
- Binds to various receptors including serotonin 5-HT3, Adenosine A1/A2, α1/α2/β adrenergic, dopamine D1/D2, GABA A/B, histamine H1, κ-opioid, mACh, and serotonin 5-HT2 receptors

Interactions
- Valproate increases Lamitrin concentrations
- Carbamazepine, phenytoin, phenobarbital, primidone, and rifampin decrease Lamitrin concentrations
- Estrogen-containing oral contraceptives decrease Lamitrin concentrations
- Protease inhibitors lopinavir/ritonavir and atazanavir/ritonavir decrease Lamitrin exposure

Pregnancy & Lactation

Precautions & Warnings
- Discontinue at the first sign of rash
- Monitor for blood dyscrasias
- Monitor for suicidal behavior and ideation
- Monitor for aseptic meningitis
Use in Special Populations
- Safety and effectiveness in very young pediatric patients aged 1 to 24 months not demonstrated
- Safety and effectiveness in pediatric patients aged 10 to 17 years not established
Overdose Effects
- Ataxia
- Nystagmus
- Seizures
- Decreased level of consciousness
- Coma
- Intraventricular conduction delay
