Indications
- Treatment of partial-onset seizures in adults
- Monotherapy for partial-onset seizures in adolescents and adults
- Adjunctive therapy for myoclonic seizures in adults and adolescents
- Management of primary generalized tonic-clonic seizures in adults and adolescents
Description
- Mechanism of action is unknown
- Minimal activity in submaximal stimulation and threshold tests
- Protection against secondarily generalized activity from focal seizures
- Inhibition of burst firing in epileptiform activity
Dosage
- Starting dose of 250mg twice daily for adults and adolescents from 16 years
- Initial therapeutic dose of 500mg twice daily for adults (≥18 years) and adolescents (12 to 17 years) weighing 50 kg or more
- Tablet formulation not suitable for use in infants and children under the age of 6 years
- Dosing adjustments for renal impairment
Administration
- Oral administration
- Tablets to be taken with sufficient quantity of liquid
- Administered in two equally divided doses
Interaction
- No significant influence on serum concentrations of existing antiepileptic medicinal products
- Caution advised when co-administered with probenecid or methotrexate
- Cautious use with laxatives such as macrogol
- No significant interaction with food and alcohol
Contraindications
- Hypersensitivity to the active substance or pyrrolidone derivatives
- Hypersensitivity to any of the excipients
Side Effects
- Common adverse reactions include somnolence, asthenia, infection, dizziness
- Pediatric patients may experience fatigue, aggression, nasal congestion, decreased appetite, irritability
Pregnancy & Lactation
- Not recommended during pregnancy and in women of childbearing potential not using contraception
- Excreted in human breast milk
Precautions & Warnings
- Dose adjustment required for renal impairment
- Rare cases of decreased blood cell counts reported
- Monitoring for signs of depression and suicidal ideation and behaviour recommended
- Caution advised in pediatric population
Use in Special Populations
- Dose adjustment recommended in elderly patients with compromised renal function
- Individualized dosing according to renal function
Overdose Effects
- Symptoms may include agitation, aggression, respiratory depression, coma
- No specific antidote
- Treatment may include symptomatic care and haemodialysis
