Indications

  • Monotherapy for partial-onset seizures
  • Adjunctive therapy for partial-onset seizures with or without generalization
  • Treatment of myoclonic seizures in Juvenile Myoclonic Epilepsy
  • Treatment of primary generalized tonic-clonic seizures

Description

  • Mechanism of antiepileptic effect is unknown
  • Minimal activity in submaximal stimulation and threshold tests
  • Protection against secondarily generalized activity from focal seizures induced by chemoconvulsants

Dosage

  • Recommended starting dose for adults and adolescents from 16 years of age is 250 mg twice daily
  • Maximum dose is 1500 mg twice daily
  • Different dosages for pediatric population based on age and weight

Administration

  • Film-coated tablets to be taken orally with or without food
  • Administered in two equally divided doses

Interaction

  • No influence on serum concentrations of existing antiepileptic drugs
  • Interactions with probenecid, methotrexate, and laxatives reported
  • Food and alcohol may slightly alter the rate of absorption

Contraindications

  • Hypersensitivity to the active substance or other pyrrolidone derivatives or to any of the excipients

Side Effects

  • Common adverse reactions include somnolence, asthenia, infection, dizziness, fatigue, aggression, nasal congestion, decreased appetite, and irritability

Pregnancy & Lactation

  • Not recommended during pregnancy and breastfeeding
  • Potential impact on fertility unclear

Precautions & Warnings

  • Dose adjustment required for renal impairment
  • Rare cases of decreased blood cell counts reported
  • Risk of suicidal thoughts and behaviors reported
  • Tablet formulation not adapted for use in infants and children under 6 years

Use in Special Populations

  • Dose adjustment in elderly patients with compromised renal function
  • Individualized dose according to renal function in renal impairment
  • No dose adjustment needed in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms of overdose include somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma
  • No specific antidote available
  • Symptomatic treatment may include haemodialysis
