Indications

  • Monotherapy for partial-onset seizures in adults and adolescents
  • Adjunctive therapy for partial-onset seizures in adults, adolescents, children, and infants
  • Treatment of myoclonic seizures in adults and adolescents with Juvenile Myoclonic Epilepsy
  • Treatment of primary generalized tonic-clonic seizures in adults and adolescents with Idiopathic Generalised Epilepsy

Mechanism of Action

  • Unknown mechanism of exerting antiepileptic effect
  • Minimal activity in submaximal stimulation and threshold tests
  • Inhibition of burst firing without affecting normal neuronal excitability

Dosage

  • Recommended starting dose of 250 mg twice daily for adults and adolescents
  • Initial therapeutic dose of 500 mg twice daily after two weeks
  • Maximum dose of 1500 mg twice daily for adults & adolescents
  • Different dosage requirements for pediatric population

Administration

Oral administration with sufficient liquid, taken with or without food

Interaction

  • No significant influence on serum concentrations of existing antiepileptic drugs
  • Concomitant administration with probenecid, methotrexate, or laxatives may require monitoring
  • No data available for interaction with alcohol

Contraindications

Hypersensitivity to active substance or other pyrrolidone derivatives

Side Effects

  • Common adverse reactions include somnolence, asthenia, infection, dizziness in adults
  • Common adverse reactions include fatigue, aggression, nasal congestion, decreased appetite in pediatric patients

Pregnancy & Lactation

  • Not recommended during pregnancy or breastfeeding unless clinically necessary
  • Excreted in breast milk, breastfeeding not recommended

Precautions & Warnings

  • Dose adjustment required in renal impairment
  • Rare cases of decreased blood cell counts associated with initial treatment
  • Monitoring for signs of depression and suicidal ideation and behaviors
  • Not adapted for use in infants and children under 6 years

Use in Special Populations

  • Dose adjustment recommended for the elderly and patients with compromised renal function

Overdose Effects

  • Observation of somnolence, agitation, aggression, depressed level of consciousness
  • No specific antidote, treatment is symptomatic
