Indications

  • Monotherapy in the treatment of partial-onset seizures with or without secondary generalisation in adults and adolescents from 16 years of age with newly diagnosed epilepsy
  • Adjunctive therapy for partial-onset seizures with or without secondary generalization in adults, adolescents, children, and infants from 1 month of age with epilepsy
  • Treatment of myoclonic seizures in adults and adolescents from 12 years of age with Juvenile Myoclonic Epilepsy
  • Treatment of primary generalized tonic-clonic seizures in adults and adolescents from 12 years of age with Idiopathic Generalised Epilepsy

Description

  • Precise mechanism(s) by which the medicine exerts its antiepileptic effect is unknown
  • Minimal activity in submaximal stimulation and in threshold tests
  • Protection against secondarily generalized activity induced by chemo convulsants that mimic features of human complex partial seizures
  • Inhibition of burst firing without affecting normal neuronal excitability
  • Selective prevention of hyper synchronization of epileptiform burst firing and propagation of seizure activity

Dosage

  • Recommended starting dose of 250 mg twice daily for adults and adolescents from 16 years of age
  • Initial therapeutic dose of 500 mg twice daily after two weeks
  • Maximum dose of 1500 mg twice daily
  • Different dosage recommendations for patients based on age, weight, and formulation (tablet or oral solution)

Administration

  • Must be taken orally
  • Swallowed with sufficient quantity of liquid
  • May be taken with or without food
  • Administered in two equally divided doses

Interaction

  • No influence on serum concentrations of existing antiepileptic medicinal products
  • No influence by antiepileptic medicinal products on the pharmacokinetics of the medicine
  • Concomitant administration with probenecid and methotrexate
  • Isolated reports of decreased efficacy when concomitantly administered with osmotic laxative macrogol

Contraindications

  • Hypersensitivity to the active substance or other pyrrolidone derivatives or to any of the excipients

Side Effects

  • Common adverse reactions including somnolence, asthenia, infection, dizziness, fatigue, aggression, nasal congestion, decreased appetite, and irritability

Pregnancy & Lactation

  • Not recommended during pregnancy and in women of childbearing potential not using contraception unless clinically necessary
  • Excreted in human breast milk
  • Breastfeeding is not recommended
  • No impact on fertility detected in animal studies, potential risk for humans unknown

Precautions & Warnings

  • Administration to patients with renal impairment may require dose adjustment
  • Rare cases of decreased blood cell counts described
  • Suicidal thoughts and behaviour reported in patients treated with anti-epileptic agents
  • Tablet formulation not adapted for use in infants and children under the age of 6 years

Use in Special Populations

  • Dose adjustment recommended in elderly patients with compromised renal function
  • Daily dose must be individualized according to renal function
  • No dose adjustment needed in patients with mild to moderate hepatic impairment

Overdose Effects

  • Observation of somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma
  • No specific antidote available
  • Symptomatic treatment recommended
  • Possible use of haemodialysis

Therapeutic Class
