Indications
- Monotherapy in the treatment of partial seizures in adults
- Adjunctive therapy in the treatment of partial seizures in adults
- Monotherapy in the treatment of partial seizures in children aged 4-16 years
- Adjunctive therapy in the treatment of partial seizures in children aged 2-16 years
Pharmacology
- Primary mechanism of action is blockade of voltage-sensitive sodium channels
- Stabilization of hyper excited neural membranes
- Inhibition of repetitive neuronal firing
- Diminishment of propagation of synaptic impulses
Dosage & Administration
- Initiate adult dosage with 600 mg/day, given twice-a-day
- Maximum recommended daily dose for adults is 2400 mg/day
- Pediatric initiation dosage is 8-10 mg/kg/day, given twice-a-day
- Maximum maintenance dose for pediatric patients aged 2 to <4 years should not exceed 60 mg/kg/day
Interaction
- Inhibits the enzyme CYP2C19
- Interactions possible with medicinal products metabolised by CYP2C19
- Potential interactions with phenobarbital and phenytoin
- Impact on hormonal contraceptives and certain cardiovascular drugs
Contraindications
- Hypersensitivity to the active substance or excipients
Side Effects
- Most commonly reported adverse reactions include somnolence, headache, dizziness, diplopia, nausea, and vomiting
- Clinically significant hyponatremia reported rarely
- Class I (immediate) hypersensitivity reactions including rash, pruritus, and urticaria
Pregnancy & Lactation
- May cause serious birth defects when administered during pregnancy
- Cautionary administration of vitamin K1 as a preventive measure in pregnancy and to the newborn
- Excreted in human breast milk, should not be used during breastfeeding
Precautions & Warnings
- Caution advised when combining with alcohol due to possible additive sedative effect
- Gradual withdrawal recommended to minimize potential of increased seizure frequency
