- Start at 50 mg three times a day (150 mg/day) for DPN, increase to 300 mg/day within 1 week based on efficacy and tolerability
- For PHN, start at 75 to 150 mg two times a day, increase to 300 mg/day within 1 week based on efficacy
- For fibromyalgia, recommended dose of 300 to 450 mg/day, start at 75 mg two times a day (150 mg/day)
