Indications

  • Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
  • Postherpetic neuralgia (PHN)
  • Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
  • Fibromyalgia
  • Neuropathic pain associated with spinal cord injury

Pharmacology

Pregabalin is a structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). It binds with high affinity to the alpha2-delta site in central nervous system tissues.

Dosage

  • Maximum recommended dose: 100 mg three times a day for neuropathic pain associated with diabetic peripheral neuropathy
  • Recommended dose for postherpetic neuralgia: 75 to 150 mg two times a day
  • Recommended dose for fibromyalgia: 300 to 450 mg/day
  • Recommended dose range for neuropathic pain associated with spinal cord injury: 150 to 600 mg/day

Administration

  • Route of administration: Oral
  • Special instruction for Pregabalin CR tablet administration: Swallowed whole after an evening meal

Interaction

  • Drug interaction with medication: Unlikely to be involved in significant pharmacokinetic drug interactions

Contraindications

Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components

Side Effects

  • Dizziness
  • Somnolence
  • Dry mouth
  • Edema
  • Blurred vision
  • Weight gain
  • Difficulty with concentration/attention (thinking abnormal)
  • Increased weight and increased appetite in pediatric patients

Pregnancy & Lactation

  • Pregnancy: No adequate and well-controlled studies, potential risk to fetus
  • Lactation: Small amounts detected in the milk, not recommended during treatment

Precautions & Warnings

  • Angioedema and hypersensitivity reactions may occur
  • Increased risk of suicidal thoughts or behavior
  • Respiratory depression may occur when used with concomitant CNS depressants
  • May cause dizziness and somnolence, impairing ability to drive or operate machinery
  • Increased seizure frequency or other adverse reactions upon rapid discontinuation
  • Possible peripheral edema
  • Caution when coadministering with thiazolidinedione antidiabetic agents

Use in Special Populations

  • Use in children and adolescents: Safety and effectiveness not established in various conditions

Overdose Effects

Signs and symptoms include reduced consciousness, depression/anxiety, seizures, and heart block. No specific antidote.
