Indications

  • Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
  • Postherpetic neuralgia (PHN)
  • Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
  • Fibromyalgia
  • Neuropathic pain associated with spinal cord injury

Pharmacology

Structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). Binds with high affinity to the alpha2-delta site in central nervous system tissues.

Dosage

  • Maximum recommended dose for neuropathic pain associated with diabetic peripheral neuropathy is 300 mg/day
  • Recommended dose for postherpetic neuralgia is 150 to 300 mg/day
  • Recommended dose for fibromyalgia is 300 to 450 mg/day
  • Recommended dose range for neuropathic pain associated with spinal cord injury is 150 to 600 mg/day
  • When switching from Pregabalin capsules to Pregabalin CR capsule, dose conversion should be done as per provided information
  • Different dose conversions for Pregabalin tablets to Pregabalin CR capsules

Administration

  • Route: Oral
  • Instructions:
    • Can be taken with or without food
    • Pregabalin CR tablet should be administered after an evening meal
    • Should be swallowed whole and should not be split, crushed or chewed
    • Gradual tapering is recommended when discontinuing the medication

Interaction

  • Medication: Unlikely to be involved in significant pharmacokinetic drug interactions

Contraindications

Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components

  • Weight gain
  • Thinking abnormal (primarily difficulty with concentration/attention)
  • Increased weight and increased appetite in pediatric patients

PregnancyAndLactation

  • Pregnancy: No adequate and well-controlled studies with pregabalin in pregnant women
  • Lactation: Small amounts of pregabalin have been detected in the milk of lactating women. Breastfeeding is not recommended during treatment with pregabalin due to potential risk of tumorigenicity

PrecautionsAndWarnings

  • Angioedema and hypersensitivity reactions may occur, requiring immediate discontinuation
  • Increased risk of suicidal thoughts or behavior
  • Respiratory depression may occur when used with concomitant CNS depressants
  • May cause dizziness, somnolence, and impair patients' ability to drive or operate machinery
  • Increased seizure frequency or adverse reactions may occur if rapidly discontinued
  • May cause peripheral edema
  • Cautious administration when coadministering with thiazolidinedione antidiabetic agents

Safety and effectiveness have not been established in pediatric patients for various indications

OverdoseEffects

Signs and symptoms include reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, and heart block. No specific antidote. General supportive care is indicated.
