Description
Pegamax is a medication used for the management of neuropathic pain associated with diabetic peripheral neuropathy, postherpetic neuralgia, spinal cord injury, fibromyalgia, and as adjunctive therapy for the treatment of partial-onset seizures in both adults and pediatric patients.
Indications
- Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
- Postherpetic neuralgia (PHN)
- Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
- Fibromyalgia
- Neuropathic pain associated with spinal cord injury
Pharmacology
- Structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA)
- Binds with high affinity to the alpha2-delta site in central nervous system tissues
Dosage
- Maximum recommended dose of Pregabalin is 100 mg three times a day (300 mg/day) for DPN in adults
- Recommended dose of Pregabalin is 75 to 150 mg two times a day or 50 to 100 mg three times a day (150 to 300 mg/day) for PHN in adults
- Recommended dose of Pregabalin is 300 to 450 mg/day for fibromyalgia in adults
- Recommended dose range of Pregabalin is 150 to 600 mg/day for neuropathic pain associated with spinal cord injury in adults
- Pregabalin CR tablet should be administered after an evening meal
Administration
- Oral route of administration
- Can be taken with or without food
- Pregabalin CR tablet should be swallowed whole and should not be split, crushed or chewed
Interaction
- Unlikely to be involved in significant pharmacokinetic drug interactions
- Not applicable for drug interaction with food and others
Contraindications
- Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components
Side effects
- Common side effects in adults: dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and thinking abnormal
- Common side effects in pediatric patients: increased weight and increased appetite for the treatment of partial onset seizures
Pregnancy and lactation
- No adequate and well-controlled studies with pregabalin in pregnant women
- Small amounts of pregabalin detected in the milk of lactating women
- Breastfeeding not recommended during treatment with pregabalin
Precautions and warnings
- Angioedema can occur and may be associated with life threatening respiratory compromise
- Discontinue immediately in case of hypersensitivity reactions
- Increase the risk of suicidal thoughts or behavior
- May cause dizziness and somnolence and impair patients ability to drive or operate machinery
- Withdraw gradually over a minimum of 1 week
- May cause peripheral edema
Use in special populations
- Safety and effectiveness not established for pediatric patients for certain indications
- Safety and effectiveness of Pegamax extended-release tablet not established in pediatric patients
Overdose effects
- Signs and symptoms include reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, and heart block
- No specific antidote
- General supportive care indicated including monitoring of vital signs and observation of the clinical status of the patient
