Composition

  • Sodium Valproate tablet - 200mg
  • Sodium Valproate syrup - 200mg/5ml
  • Sodium Valproate controlled release 200 tablet - 200mg Sodium Valproate BP
  • Sodium Valproate controlled release 300 tablet - 300mg Sodium Valproate BP
  • Sodium Valproate controlled release 500 tablet - 500mg Sodium Valproate BP
  • Sodium Valproate injection - 500mg Valproic Acid

Pharmacology

  • Anti-epileptic activity against a variety of seizures
  • Mechanism related to increase brain levels of gamma-aminobutyric acid (GABA)

Dosage & Administration

  • Oral administration
  • Initial adult dosage - 600mg daily
  • Maximum adult dosage - 2.5g daily
  • Initial pediatric dosage - 400mg daily
  • Maximum pediatric dosage - 35mg/kg daily
  • Prophylaxis of febrile convulsion dosage - 20-30mg/kg daily
  • Bipolar disorder dosage - 600mg daily
  • Prophylaxis of migraine dosage - 400-600mg daily
  • Injection administration
  • Adult injection dosage - 400-800mg
  • Maximum adult injection dosage - 2500mg/day
  • Pediatric injection dosage - 20-30mg/kg/day
  • Maximum pediatric injection dosage - 40mg/kg/day

Interaction

  • Non-specific inhibitor of drug metabolism
  • Significant interactions with Phenobarbital, Phenytoin, Warfarin, Aspirin

Contraindications

  • Known hypersensitivity to the drug
  • Liver dysfunction
  • Restricted use during pregnancy and in women of childbearing potential

Side Effects

  • Anorexia, nausea, vomiting
  • Effects on CNS - sedation, ataxia, tremor
  • Rare occurrence of rash, alopecia, increased appetite
  • Elevation of liver enzymes in plasma in up to 40% of patients
  • Fulminant hepatitis (rare)
  • Hepatic injury in children below 2 years and those on multiple antiepileptic agents
  • Acute pancreatitis
  • Hyperammonemia

Pregnancy & Lactation

  • Crosses the placenta
  • Associated with neural tube defects in newborns
  • Excreted in breast milk

Precautions & Warnings

  • Monitoring of liver functions
  • Ensuring no potential for bleeding before starting and major surgery
  • Caution in renal impairment, pregnancy, and breast-feeding
  • Partial elimination in urine as a ketone metabolite
  • Avoid sudden withdrawal of therapy
