Medicine Details

Indications

  • Treatment of all types of epilepsy
  • Prophylaxis of febrile convulsion
  • Prophylaxis of post-traumatic epilepsy
  • Treatment of bipolar disorder
  • Prophylaxis of migraine

Composition

  • Enteric-coated tablet contains Sodium Valproate BP 200 mg
  • Each 5 ml syrup contains Sodium Valproate BP 200 mg
  • Controlled release tablet contains Sodium Valproate 200 mg as Sodium Valproate BP 133.2 mg & Valproic acid BP 58 mg
  • Injection contains Sodium Valproate BP eqv. to Valproic Acid 500 mg

Pharmacology

  • Anti-epileptic activity against a variety of seizures
  • Increase brain levels of gamma-aminobutyric acid (GABA)

Dosage & Administration

  • May be given once or twice daily
  • Initial dose for adults: 600 mg daily in divided doses
  • Maximum maintenance dose for adults: 2.5 g daily
  • Initial dose for children (over 20 kg): 400 mg daily
  • Maximum dose for children (over 20 kg): 35 mg/kg daily
  • Initial dose for children (up to 20 kg): 20 mg/kg daily
  • Dosage for febrile convulsion: 20-30 mg/kg/day in 3 divided doses
  • Initial dose for bipolar disorder: 600 mg daily
  • Maintenance dosage for bipolar disorder: 1000-2000 mg daily
  • Dosage for prophylaxis of migraine: 400-600 mg daily

Interaction

  • Acts as a non specific inhibitor of drug metabolism
  • Significantly interacts with Phenobarbital, Phenytoin, Warfarin, Aspirin

Contraindications

  • Known hypersensitivity to the drug
  • Liver dysfunction
  • Restricted use during pregnancy and in women of childbearing potential

Side Effects

  • Common: anorexia, nausea, vomiting
  • Effects on CNS: sedation, ataxia, tremor
  • Occasional: rash, alopecia, stimulation of appetite
  • Hepatic function effects: elevation of liver enzymes in plasma, fulminate hepatitis, acute pancreatitis, hyperammonemia

Pregnancy & Lactation

  • Crosses the placenta
  • Exposure associated with neural tube defects
  • Excreted in breast milk, but probably causes no risk to the child

Precautions & Warnings

  • Liver function monitoring
  • No undue potential for bleeding
  • Care in renal impairment, pregnancy, breast-feeding, systemic lupus erythematosus
  • False interpretation of urine ketone test due to ketone metabolite in urine
  • Avoid sudden withdrawal of therapy
