Composition

  • Enteric-coated tablet containing Sodium Valproate BP 200 mg
  • Syrup containing Sodium Valproate BP 200 mg
  • Controlled release tablet containing Sodium Valproate 200 mg as Sodium Valproate BP 133.2 mg & Valproic acid BP 58 mg
  • Controlled release tablet containing Sodium Valproate 300 mg as Sodium Valproate BP 199.8 mg & Valproic acid BP 87 mg
  • Controlled release tablet containing Sodium Valproate 500 mg as Sodium Valproate BP 333 mg & Valproic acid BP 145 mg
  • Injectable solution containing Sodium Valproate BP eqv. to Valproic Acid 500 mg

Pharmacology

  • Anti-epileptic activity against a variety of seizures
  • Increase brain levels of gamma-aminobutyric acid (GABA)

Dosage & Administration

  • Oral administration
  • Initial dose for adults: 600 mg daily, increased by 200 mg/day at 3-day intervals
  • Usual maintenance dose for adults: 1-2 g daily
  • Initial dose for children (over 20 kg): 400 mg daily
  • Maximum dose for children (over 20 kg): 35 mg/kg daily
  • Initial dose for children (up to 20 kg): 20 mg/kg daily
  • Febrile convulsion dosage: 20-30 mg/kg/day in 3 divided doses
  • Bipolar disorder initial dosage: 600 mg daily, maintenance dosage: 1000-2000 mg daily
  • Prophylaxis of migraine dosage: 400-600 mg daily, some may require 1000-1500 mg daily
  • Injection administration for adults: usually 400-800 mg, followed by continuous or repeated infusion up to a maximum of 2500 mg/day
  • Injection administration for children: Initial 20-30 mg/kg/day, maximum 40 mg/kg/day

Interaction

  • Non-specific inhibitor of drug metabolism
  • Significant interaction with Phenobarbital, Phenytoin, Warfarin, Aspirin

Contraindications

  • Known hypersensitivity to the drug
  • Liver dysfunction
  • Restriction during pregnancy and in women of childbearing potential

  • Anorexia
  • Nausea
  • Vomiting
  • CNS effects: sedation, ataxia, tremor
  • Rash
  • Alopecia
  • Stimulation of appetite
  • Elevation of liver enzymes in plasma
  • Fulminate hepatitis (rare)
  • Hepatic injury in children below 2 years with other medical conditions
  • Acute pancreatitis
  • Hyperammonemia

Pregnancy & Lactation

  • Crosses the placenta
  • Associated with neural tube defects in newborn
  • Excreted in breast milk

Precautions & Warnings

  • Monitoring of liver functions before therapy and during first 6 months
  • No undue potential for bleeding before starting and before major surgery
  • Care should be taken in renal impairment, pregnancy, breast-feeding, and systemic lupus erythematosus
  • Partially eliminated in the urine as a ketone metabolite, leading to false interpretation of the urine ketone test
  • Sudden withdrawal of therapy should be avoided
