Indications
- Monotherapy for newly diagnosed epilepsy in adults and children
- Adjunctive therapy for inadequately controlled epilepsy
- Prophylaxis of migraine headache
Pharmacology
- Reduction of neuronal excitability to prevent seizures
- Action on voltage-dependent sodium channels, GABA receptors, and glutamate receptors
Dosage & Administration
- Titration for adults and children
- Recommended initial target doses for monotherapy and adjunctive therapy
- Review of continuing therapy every six months
Interaction
- Effect of Topmate on steady-state plasma concentrations of other antiepileptic drugs
- Caution when used with CNS depressants, oral contraceptives, lithium, hydrochlorothiazide, metformin
- Monitoring required when used concomitantly with pioglitazone, glibenclamide, and valproic acid
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Nausea, abdominal pain, dyspepsia, diarrhoea
- Taste disturbance, weight loss, paraesthesia
- Headache, fatigue, dizziness, insomnia, anxiety, depression
- Visual disturbance, impaired memory & concentration
- Rarely: sucidal ideation, reduced sweetening, metabolic acidosis, alopecia, Leucopenia, thrombocytopenia, serious skin reaction
Pregnancy & Lactation
- Not to be used during pregnancy or breastfeeding
- Physician's opinion required to outweigh potential risk to the foetus
Precautions & Warnings
- Gradual withdrawal in patients with history of seizures or epilepsy
- Potential for sedation and visual disturbances/blurred vision
- Advice against driving, operating machinery, or risky activities in affected patients
Overdose Effects
- Severe metabolic acidosis
- Immediate stomach emptying, activated charcoal, supportive treatment
- Haemodialysis for effective removal from the body
