Indications

• Treatment of Parkinson's disease and syndrome
• Relief of symptoms of parkinsonism
• Management of tremor and dysphagia
• Use before physiotherapy for motor recovery after stroke

Description

• Levodopa crosses blood-brain barrier and is converted to dopamine
• Carbidopa reduces peripheral metabolism of Levodopa to dopamine

Dosage & Administration

• Initiation and titration of dosage
• Different dosage for 100/10 mg and 250/25 mg tablets
• Individualized maintenance dose
• Limitation of total daily dosage of Carbidopa

Interaction

• Postural hypotension with antihypertensives
• Adverse reactions with tricyclic antidepressants
• Decrease in bioavailability with ferrous sulphate or ferrous gluconate
• Reduced therapeutic effects with dopamine-2 receptor antagonists and isoniazid
• Potential loss of therapeutic response with phenytoin and papaverine
• Severe orthostatic hypotension with selegiline

Contraindications

• Hypersensitivity to Carbidopa and Levodopa
• Narrow-angle glaucoma
• Potential activation of malignant melanoma

Side Effects

• Central neuropharmacologic adverse effects
• Dosage reduction to diminish adverse reactions
• Common adverse effects and their categories

Pregnancy & Lactation

• Unknown effects on human pregnancy
• Visceral and skeletal malformations in rabbits
• Consideration of benefits and hazards in women of childbearing potential
• Unknown excretion of Carbidopa in human milk
• Decision between nursing and medicine use

Precautions & Warnings

• Not recommended for medicine-induced extrapyramidal reactions
• Careful observation for depression with suicidal tendencies
• Caution in patients with cardiovascular, pulmonary, hepatic, renal, or endocrine disease
• Monitoring of cardiac function in patients with myocardial infarction
• Caution in patients with wide-angle glaucoma

Use in Special Populations

• Not recommended for use in patients below the age of 18 years
