Indications

• Treats Parkinson's disease and syndrome
• Relieves symptoms of parkinsonism
• Manages tremor, dysphagia, sialorrhea
• Useful in postural instability associated with Parkinson's

Description

• Levodopa serves as metabolic precursor of dopamine
• Carbidopa inhibits peripheral metabolism of Levodopa

Dosage and administration

• Initiate with 100/10 mg tablet
• Titrate upwards for optimum carbidopa dosage
• For patients starting with 250/25 mg tablet, adjust dosage accordingly
• Individualized therapy for desired response
• Maximum daily dosage of Carbidopa is limited to 200mg

Interaction

• May require adjustment of antihypertensive medicine
• Potential reduction in therapeutic effects with dopamine-2 receptor antagonists
• Severe orthostatic hypotension with selegiline

Contraindications

• Hypersensitivity to Carbidopa and Levodopa
• Narrow-angle glaucoma
• Suspected undiagnosed skin lesions or history of melanoma

Side effects

• Central neuropharmacologic activity of dopamine
• Diminish adverse effects through dosage reduction
• Common adverse effects: dyskinesias, nausea, chest pain, anorexia, orthostatic effects, dark saliva, dizziness, somnolence, hallucinations, dark sweat

Pregnancy and lactation

• Unknown effects on human pregnancy
• Potential malformations in rabbits
• Consider benefits against possible hazards in women of childbearing potential

Precautions and warnings

• Not recommended for medicine-induced extrapyramidal reactions
• Observation for depression with suicidal tendencies
• Caution in patients with severe cardiovascular, pulmonary, renal, hepatic, endocrine disease

Use in special populations

• Not recommended for patients below 18 years
