Indications
- Treatment of Parkinson's disease and syndrome
- Relief of symptoms of parkinsonism
- Management of tremor, dysphagia, sialorrhea
- Use before physiotherapy to increase motor recovery after stroke
Description
- Levodopa crosses the blood-brain barrier and is converted to dopamine in the brain
- Carbidopa inhibits peripheral metabolism of Levodopa to dopamine
Dosage & Administration
- Initiation dosage for 100/10 mg tablet and 250/25 mg tablet
- Titration process for reaching optimum dosage
- Individualized therapy for maintenance dose
Interaction
- Potential interaction with antihypertensive medicines
- Possible adverse reactions with tricyclic antidepressants
- Decrease in bioavailability when ingested with ferrous sulphate or isoniazid
- Reversal of therapeutic effects by phenytoin and papaverine
- Severe orthostatic hypotension with selegiline
Contraindications
- Hypersensitivity to Carbidopa and Levodopa
- Narrow-angle glaucoma
- Presence of suspicious undiagnosed skin lesions or history of melanoma
Side Effects
- Central neuropharmacologic adverse effects of dopamine
- Diminishable reactions with dosage reduction
- Various adverse effects including dyskinesias, nausea, syncope
- Cardiovascular, gastrointestinal, haemotologic, hypersensitivity, nervous system, respiratory, skin, urogenital effects
Pregnancy & Lactation
- Unknown effects on human pregnancy
- Possibility of causing malformations in rabbits
- Consideration of benefits vs. possible hazards in women of childbearing potential
- Unknown excretion in human milk
- Decision making for nursing mothers
Precautions & Warnings
- Not recommended for medicine-induced extrapyramidal reactions
- Careful observation for depression with suicidal tendencies
- Cautious administration for patients with severe cardiovascular or pulmonary disease
- Monitoring cardiac function in patients with history of myocardial infarction
- Cautious treatment for patients with chronic wide-angle glaucoma

- Not recommended for use in patients below the age of 18 years
