Indications

  • Treatment of Parkinson's disease and syndrome
  • Relief of rigidity and bradykinesia
  • Management of tremor, dysphagia, sialorrhea, and postural instability
  • Increases motor recovery after stroke

Description

  • Levodopa crosses the blood-brain barrier and is converted to dopamine
  • Carbidopa reduces peripheral metabolism of Levodopa to dopamine

Dosage & Administration

  • Dosage initiation with 100/10 mg tablet
  • Titration upward to achieve optimum dosage of carbidopa
  • Initial dose for 250/25 mg tablet
  • Dosage adjustment for patients taking more than 1500 mg of Levodopa a day
  • Maintenance dose individualization
  • Substitution with 250/25 mg tablet
  • Limited experience with total daily dosage greater than 200 mg Carbidopa

Interaction

  • Symptomatic postural hypotension with antihypertensive medicines
  • Adverse reactions with tricyclic antidepressants
  • Decrease in bioavailability when ingested with ferrous sulphate or ferrous gluconate
  • Reduced therapeutic effects with dopamine-2 receptor antagonists and isoniazid
  • Reversal of beneficial effects by phenytoin and papaverine
  • Severe orthostatic hypotension with concomitant therapy with selegiline

Contraindications

  • Hypersensitivity to Carbidopa and Levodopa
  • Narrow-angle glaucoma
  • Risk of activating malignant melanoma

Side Effects

  • Central neuropharmacologic activity of dopamine
  • Reduction of reactions by dosage reduction
  • Adverse effects such as dyskinesias and nausea
  • Frequent adverse effects in various bodily systems

Pregnancy & Lactation

  • Unknown effects on human pregnancy
  • Visceral and skeletal malformations in rabbits
  • Consideration of benefits and hazards for women of childbearing potential
  • Unknown excretion in human milk

Precautions & Warnings

  • Not recommended for medicine-induced extrapyramidal reactions
  • Observation for development of depression with suicidal tendencies
  • Cautious treatment of patients with various diseases and conditions
  • Monitoring of cardiac function in patients with myocardial infarction

Use in Special Populations

  • Safety and effectiveness not established in infants and children below 18 years
