Indications
- adjunctive treatment of all forms of parkinsonian syndrome
- symptomatic treatment of idiopathic, postencephalitic and arteriosclerotic parkinsonian
- control of troublesome extrapyramidal symptoms
- treatment of pseudoparkinsonian, acute dystonic reactions and akathisia induced by neuroleptic drugs
Pharmacology
- antimuscarinic antiparkinsonian agent
- synthetic tertiary amine
- correction of relative cholinergic excess
- rapid absorption from the G.I. tract
- onset of action within 5 to 20 minutes
- duration of effect up to 4 hours
Dosage & Administration
- Tablet administered orally, preferably after meals
- initial dosage for parkinsonism: 2.5 mg 3 times a day, with a maximum total daily dose of 20 to 30 mg
- initial dosage for drug induced extrapyramidal symptoms: 2.5 mg 3 times a day, with incremental increases as needed
- IM or IV injection: 5-10 mg, repeated if necessary; maximum 20 mg daily
Interaction
- increased anticholinergic activity with certain agents
- reduced absorption of ketoconazole when administered concomitantly
Contraindications
- caution in children and geriatric patients
- caution in patients with diarrhoea, cardiovascular disease, glaucoma, urinary retention, hepatic or renal impairment
- unestablished safety during pregnancy and lactation
- no data available on drug excretion in breast milk
Side Effects
- dryness of the mouth at usual dosage levels
- mydriasis and blurred vision occasionally
- adverse G.I. effects including nausea, vomiting, epigastric distress, constipation
- occasional allergic reactions or muscular weakness
- possible vertigo, confusion, and hallucination at high doses
- adverse effects usually minimized by dosage adjustment and administration after meals

Precautions & Warnings
- consideration for hepatic & renal impairment, children, elderly, pregnancy, and lactation
- risk of psychotic episode precipitation in patients with mental disorders
