Indications
- Adjunctive treatment of all forms of parkinsonian syndrome
- Symptomatic treatment of idiopathic (paralysis agitants), postencephalitic and arteriosclerotic parkinsonian
- Control of troublesome extrapyramidal symptoms including pseudoparkinsonian, acute dystonic reactions and akathisia induced by neuroleptic drugs
Pharmacology
- Antimuscarinic antiparkinsonian agent
- Corrects relative cholinergic excess in parkinsonian due to dopamine deficiency
- Rapidly absorbed from G.I. tract
- Intravenous administration onset of action within 5 to 20 minutes with effect lasting up to 4 hours
Dosage & Administration
- Oral administration
- Tablet form
- Normal dosage for Parkinsonism: 2.5 mg 3 times a day, then 5 mg 3 times a day and occasionally 5 mg at bedtime, Drug induced extrapyramidal symptom: Initially 2.5 mg 3 times a day, IM or IV injection: 5-10 mg, repeated if necessary after 20 minutes; maximum 20 mg daily can be given
Interaction
- Increased anticholinergic activity with certain agents
- Reduced absorption of ketoconazole with concomitant administration
Contraindications
- Caution in children, geriatric patients, patients with diarrhoea, cardiovascular disease, glaucoma, urinary retention, hepatic or renal impairment
- Not established for use during pregnancy or lactation
Side Effects
- Dryness of the mouth
- Mydriasis
- Blurred vision
- G.I. effects (nausea, vomiting, constipation)
- Allergic reaction (e.g. rash)
- Muscular weakness
- Vertigo
- Confusion and hallucination at high doses
Pregnancy & Lactation
- Use during pregnancy, lactation or in women of childbearing age must weigh potential benefits against possible hazards to the mother and child
Precautions & Warnings
- Take precautions in case of hepatic and renal impairment, children, elderly, pregnancy, and lactation
- Avoid abrupt withdrawal to prevent rebound Parkinsonism symptoms
