Indications
- Adjunctive treatment of all forms of parkinsonian syndrome
- Symptomatic treatment of idiopathic, postencephalitic and arteriosclerotic parkinsonian
- Control of extrapyramidal symptoms induced by neuroleptic drugs
Pharmacology
- Antimuscarinic antiparkinsonian agent
- Synthetic tertiary amine
- Exerts antiparkinsonian effect by correcting relative cholinergic excess
- Rapid absorption from G.I. tract
- Intravenous administration onset within 5 to 20 minutes
- Duration of effect up to 4 hours
Dosage & Administration
- Oral administration, preferably after meals
- Initial Parkinsonism dosage: 2.5 mg 3 times a day, adjusted up to 20 to 30 mg daily
- Initial extrapyramidal symptom dosage: 2.5 mg 3 times a day, increased by 2.5 mg per day as needed
- IM or IV injection: 5-10 mg, maximum 20 mg daily
Interaction
- Potential increased anticholinergic activity with certain agents
- Reduction in ketoconazole absorption with concomitant administration
Contraindications
- Caution in children and geriatric patients
- Cautious use in patients with specific conditions such as diarrhoea, cardiovascular disease, glaucoma, urinary retention, hepatic or renal impairment
- Safety during pregnancy and lactation not established
Side Effects
- Dryness of the mouth at usual dosage levels
- Occasional mydriasis, blurred vision, G.I. effects, allergic reactions, muscular weakness
- Higher doses may cause vertigo, confusion, hallucination
- Adverse effects minimized by dosage adjustment and administration after meal
Pregnancy & Lactation
- Safe use in pregnancy, lactation, or women of childbearing age requires weighing potential benefits against possible hazards
Precautions & Warnings
- Hepatic & renal impairment precautions
- Elderly, pregnancy, and lactation precautions
- Risk of psychotic episode precipitation in patients with mental disorders
- Avoid abrupt withdrawal to prevent rebound Parkinsonism symptoms
