Indications
- Adjunct in the treatment of all forms of parkinsonism (postencephalitic, arteriosclerotic, and idiopathic)
- Useful as adjuvant therapy when treating parkinsonism with levodopa
- Control of extrapyramidal disorders caused by central nervous system drugs
Description
- Exerts a direct inhibitory effect upon the parasympathetic nervous system
- Has a relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist with higher affinity to the M1 subtype
- Higher affinity for central muscarinic receptors located in the cerebral cortex
- May modify nicotinic acetylcholine receptor neurotransmission
- May lead indirectly to enhanced dopamine release in the striatum
Dosage & Administration
- Should be individualized
- Initial dose should be low and then increased gradually
- Dosing may depend on the patient's reaction and age
- Abrupt withdrawal should be avoided
- Initial therapy for parkinsonism
- Dosage for drug-induced parkinsonism
- Concomitant use with Levodopa
- High doses may be divided into 4 parts with 3 doses administered at mealtimes and the fourth at bedtime
- Safety and effectiveness in pediatric patients have not been established
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Potential intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk for the development of tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics
Contraindications
- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

- Urinary hesitancy or retention
- Tachycardia
- Dilation of the pupil
- Increased intraocular pressure
- Choreiform movements
- Weakness
- Vomiting
- Headache
- Exacerbation of parkinsonism with abrupt treatment withdrawal
- Neuroleptic malignant syndrome with abrupt treatment withdrawal
- Blindness due to narrow angle glaucoma
Pregnancy & Lactation
- No controlled data in animal or human pregnancy
- Recommended for use during pregnancy only when need has been clearly established and benefit outweighs risk
- Caution should be exercised when administered to a nursing woman
- May cause suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Caution in hot weather and with other atropine-like drugs
- Anhidrosis may occur more readily in certain conditions
- Severe anhidrosis and fatal hyperthermia have occurred with the use of anticholinergics
- Potential blindness due to aggravation of narrow angle glaucoma
- Neuroleptic Malignant Syndrome can occur with dose reduction or discontinuation
Overdose Effects
- Doses up to 300 mg (5 mg/kg) have been ingested without fatalities or sequelae
- Rare cases of death associated with Hexinor overdosages
- Typical central symptoms of atropine intoxication
- Treatment involves symptomatic and supportive therapy
