Product Description

Pharmacological Class
- Muscarinic acetylcholine receptor antagonist
Indications
- Adjunct in the treatment of parkinsonism
- Useful as adjuvant therapy with levodopa
- Control of extrapyramidal disorders caused by CNS drugs
Mode of Action
- Direct inhibitory effect on the parasympathetic nervous system
- Relaxing effect on smooth musculature
- Higher affinity for central muscarinic receptors
Dosage Administration
- Individualized dosage
- Low initial dose, gradually increased
- Postencephalitic patients may prefer taking it after meals
- Abrupt withdrawal should be avoided
- Initial therapy for parkinsonism: 1 mg administered on the first day, increased by 2 mg increments
- Total daily dose range for idiopathic parkinsonism: 6 to 15 mg
- Total daily dosage for drug-induced parkinsonism: 5 to 15 mg
- Concomitant use with levodopa may require reduction in dose
- High doses best tolerated when divided into 3 or 4 parts
Interactions
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Risk of tardive dyskinesia with neuroleptics
- Possible need for reduction in levodopa or dosage of other parasympathetic inhibitors

- Hypersensitivity to trihexyphenidyl
- Narrow angle glaucoma

Overdose Effects
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs
- Stupor, coma, paralysis, cardiac and respiratory arrest
- Symptomatic and supportive therapy
- Gastric lavage or methods to limit absorption
- Administration of diazepam or a short-acting barbiturate
- Respiratory support, artificial respiration, vasopressor agents
- Reversal of hyperpyrexia, fluid volume replacement, acid-balance maintenance
