Description
- Exerts direct inhibitory effect on the parasympathetic nervous system
- Relaxing effect on smooth musculature
- Inhibitory effect on parasympathetic nervous system
Pharmacology
- Non-selective muscarinic receptor antagonist
- Binds with higher affinity to M1 subtype
- Higher affinity for central muscarinic receptors in the cerebral cortex
- Lower affinity for peripheral muscarinic receptors
- May modify nicotinic acetylcholine receptor neurotransmission
- Enhanced dopamine release in the striatum
- Mechanism of action not fully elucidated
Dosage & Administration
- Individualized dosage
- Low initial dose
- Gradual dose increase, especially in patients over 60
- Timing of administration with or after meals
- Preferable administration before meals if excessive dryness of mouth occurs
- Potential need for atropine in postencephalitic patients
- Potential adverse reactions with abrupt withdrawal
- Dosage ranges for idiopathic and drug-induced parkinsonism
- Reduced dosage when used concomitantly with levodopa or other parasympathetic inhibitors
- Best tolerated when divided into doses and taken at mealtimes
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Risk of tardive dyskinesia with concomitant administration of anticholinergics and neuroleptics
- Potential need for reduced dose of levodopa during concomitant therapy

- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

- Use during pregnancy when need is clearly established

Precautions & Warnings
- Gonioscope evaluation prior to therapy
- Monitoring of intraocular pressures
- Possibility of angle closure with an increase in intraocular pressure
- Administration with caution in hot weather
- Potential for anhidrosis and hyperthermia
- Consideration of NMS with dose reduction or discontinuation
- Clinical manifestations of NMS
Overdose Effects
- Ingested doses up to 300 mg without fatalities
- Rare cases of death associated with overdosage in conjunction with other CNS-depressant agents
