Indications
- Treatment of obesity in adults with BMI > 30 kg/m^2
- Treatment of overweight patients with associated risk factors
- Treatment of obese adolescents
- Use in conjunction with hypocaloric diet
- Discontinuation after 12 weeks if weight loss is inadequate
Pharmacology
- Potent lipase inhibitor
- Forms covalent bond with gastric and pancreatic lipases
- Reduces absorption of dietary fats
- Caloric deficit for weight control
- Systemic absorption not required
Dosage & Administration
- Recommended dose of 120 mg before, during, or after main meal
- Omission of dose if meal contains no fat
- No additional benefit above 120 mg three times daily
- Increase in fecal fat within 24-48 hours after dosing
Interaction
- Reduction in cyclosporine plasma levels
- Inhibited absorption of vitamin E
- Unknown effect on absorption of vitamin D, vitamin A, and vitamin K
- Possible hypothyroidism with concurrent levothyroxine use
- Possible decrease in vitamin K absorption with warfarin use
- Potential changes in frequency and severity of convulsions with antiepileptic drugs
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to orlistat or capsule ingredients
Side Effects
- Common: gastrointestinal effects (oily spotting, flatulence, fecal urgency, abdominal discomfort)
- Rare: influenza, anxiety, headache, fatigue
- Rare cases of hypersensitivity (pruritus, exanthema, urticaria, angioedema, anaphylaxis)
Pregnancy & Lactation
- Contraindicated during pregnancy (Category X drug)
- No potential benefit to pregnant women
- Maternal weight loss may harm fetus
- Caution during lactation, unknown presence in human milk
Precautions & Warnings
- Exclusion of organic causes of obesity
- Avoid coadministration with cyclosporine
- Closely monitor antidiabetic drug treatment
- Adherence to dietary recommendations
- Encourage multivitamin supplementation
- Distribution of daily fat intake between three main meals
Overdose Effects
- 700 mg single dose and 400 mg three times daily for 15 days studied without significant adverse effects
- Observation recommended for significant overdose
- Reversible systemic effects
