Indications
- Treatment of obese patients with BMI >30 kg/m^2
- Treatment of overweight patients with BMI >28 kg/m^2 and associated risk factors
- Treatment of obese adolescents when diet and physical activity are insufficient
Pharmacology
- Potent, specific, and long-acting lipase inhibitor
- Forms a covalent bond with gastric and pancreatic lipases
- Inactivated enzymes unable to hydrolyze dietary fats
- Caloric deficit leads to weight control
- Systemic absorption not required for activity
Dosage & Administration
- Recommended dose of 120 mg capsule to be taken with main meals
- Effect results in increase in fecal fat 24-48 hours after dosing
- Safety & efficacy investigated in clinical studies lasting up to 4 years
- Special dosage instructions for elderly patients and those with hepatic/renal impairments
Interaction
- Reduction in cyclosporine plasma levels when coadministered with Slimfast
- Absorption inhibition of vitamin E acetate supplement
- Unknown effect on absorption of supplemental vitamin D, vitamin A, and nutritionally-derived vitamin K
- Reported hypothyroidism in patients treated concomitantly with Slimfast and levothyroxine
- Possible decrease in vitamin K absorption with Slimfast in patients on chronic stable doses of warfarin
- Reported convulsions in patients treated concomitantly with Slimfast and antiepileptic drugs
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to orlistat or other capsule ingredients
Side Effects
- Commonly gastrointestinal side effects such as oily spotting, flatulence, fecal urgency, and abdominal discomfort
- Rare cases of influenza, anxiety, headache, fatigue, and hypersensitivity reported
Pregnancy & Lactation
- Contraindicated during pregnancy due to potential fetal harm
- Caution advised during breastfeeding
Precautions & Warnings
- Exclusion of organic causes of obesity before prescribing Slimfast
- Coadministration precautions with cyclosporine and antidiabetic drugs
