Indications
- Treatment of obese patients with BMI >30 kg/m2
- Treatment of overweight patients with BMI >28 kg/m2 and associated risk factors such as type II diabetes, hyperlipidemia, and hypertension
- Treatment in adolescents if adequate weight reduction cannot be achieved through diet and physical activity
Pharmacology
- Potent, specific, and long-acting lipase inhibitor
- Forms covalent bond with active serine site of gastric and pancreatic lipases
- Inactivated enzyme renders dietary fats in the form of triglycerides unabsorbable
- Caloric deficit results in positive effect on weight control
- Systemic absorption not needed for activity
Dosage & Administration
- Recommended dose of 120 mg capsule taken before, during, or up to one hour after each main meal
- Omit dose if meal is missed or contains no fat
- Additional benefits not shown with doses above 120 mg three times daily
- Increase in fecal fat 24-48 hours after dosing
- Clinical studies lasting up to 4 years
- Same recommended dose for adolescents as for adults
- Special dosage instructions for elderly patients, hepatic impairment, and/or renal impairment
Interaction
- Reduction in cyclosporine plasma levels when coadministered
- Inhibition of absorption of vitamin E acetate supplement
- Effect on absorption of supplemental vitamin D, vitamin A, and nutritionally-derived vitamin K not known
- Reported cases of hypothyroidism when treated concomitantly with levothyroxine
- Potential decrease in vitamin K absorption
- Monitoring for changes in coagulation parameters in patients on warfarin
- Reported cases of convulsions when treated concomitantly with antiepileptic drugs
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to orlistat or any of the other ingredients of the capsules
Side Effects
- Common gastrointestinal side effects such as oily spotting, flatulence, fecal urgency, abdominal discomfort
- Rare cases of influenza, anxiety, headache, fatigue
- Rare cases of hypersensitivity with clinical symptoms including pruritus, exanthema, urticaria, angioedema, and anaphylaxis
Pregnancy & Lactation
- Contraindicated during pregnancy due to potential harm to fetus
- Caution advised when administered to nursing women
Precautions & Warnings
- Exclusion of organic causes of obesity before prescription
- Coadministration with cyclosporine not recommended
- Monitoring of antidiabetic drug treatment during therapy
- Adherence to dietary recommendations for minimizing gastrointestinal side effects
- Encouragement of multivitamin supplement intake
- Possible need for adjustment of oral antidiabetics or insulin dose
- Adherence to fat distribution between three main meals
- Closely monitored cyclosporine level
- Observation for 24 hours in case of significant overdose
Overdose Effects
- Single and multiple doses studied without significant adverse findings
- Systemic effects attributable to lipase-inhibiting properties should be rapidly reversible based on studies
