Indications
- Treatment of obese patients with a body mass index (BMI) >30 kg/m2
- Treatment of overweight patients with associated risk factors
- Treatment of obese adolescents
- Adjunct to a mildly hypocaloric diet for weight loss
Pharmacology
- Potent, specific, and long-acting lipase inhibitor
- Forms covalent bond with the active serine site of gastric and pancreatic lipases
- Exerts therapeutic activity in the stomach and upper small intestine
- Reduces dietary fat absorption by approximately 30%
Dosage & Administration
- Recommended dose of 120 mg capsule
- To be taken immediately before, during or up to one hour after each main meal
- Increase in fecal fat 24-48 hours after dosing
- Special dosage instructions for elderly patients and those with hepatic and/or renal impairments
Interaction
- Reduction in cyclosporine plasma levels when coadministered with Orlistat
- Inhibition of vitamin E acetate supplement absorption
- Effect on the absorption of supplemental vitamin D, vitamin A, and nutritionally-derived vitamin K is not known
- Potential interaction with levothyroxine and antiepileptic drugs
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to Orlistat or its ingredients
Side Effects
- Common gastrointestinal side effects
- Rare cases of influenza, anxiety, headache, and fatigue
- Rare cases of hypersensitivity with clinical symptoms like pruritus, exanthema, urticaria, angioedema, and anaphylaxis
Pregnancy & Lactation
- Pregnancy Category X drug
- Contraindicated during pregnancy
- Caution advised during lactation
Precautions & Warnings
- Exclusion of organic causes of obesity before prescribing Orlistat
- Avoid coadministration with cyclosporine
- Close monitoring for type II diabetic patients
- Adherence to dietary recommendations
- Encouragement for multivitamin supplement intake
Overdose Effects
- 800 mg single dose and 400 mg three times a day for 15 days studied without significant adverse findings
- Observation for 24 hours in case of significant overdose
