Indications
- Treatment of obese patients with a body mass index (BMI) > 30 kg/m^2
- Treatment of overweight patients with associated risk factors such as type II diabetes, hyperlipidemia, and hypertension
- Treatment of obese adolescents if adequate weight reduction cannot be achieved by diet and physical activity
Pharmacology
- Potent, specific, and long-acting lipase inhibitor
- Forms a covalent bond with the active serine site of gastric and pancreatic lipases
- Inactivated enzyme unable to hydrolyze dietary fats into absorbable free fatty acids and monoglycerides
- Does not require systemic absorption for activity
Dosage & Administration
- Recommended dose of 120 mg capsule to be taken before, during, or up to one hour after each main meal
- Omission of dose if a meal is missed or contains no fat
- Increase in fecal fat 24-48 hours after dosing
- Safety & efficacy investigated in clinical studies lasting up to 4 years
Interactions
- Reduction in cyclosporine plasma levels when coadministered with Orlistat
- Inhibition of absorption of vitamin E acetate supplement
- Impact on absorption of supplemental vitamin D, vitamin A, and nutritionally-derived vitamin K unknown
- Possible hypothyroidism when concomitantly treated with levothyroxine
- Possible decrease in vitamin K absorption and changes in coagulation parameters in patients on warfarin
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to Orlistat or any of the other ingredients of the capsules
Side Effects
- Common gastrointestinal side effects like oily spotting, flatulence, fecal urgency, abdominal discomfort
- Rare occurrences of influenza, anxiety, headache, fatigue
- Rare cases of hypersensitivity reported
Pregnancy & Lactation
- Contraindicated during pregnancy
- Potential hazard of maternal weight loss to the fetus
- Unknown presence in human milk
Precautions & Warnings
- Exclusion of organic causes of obesity before prescribing
- Avoid coadministration with cyclosporine
- Close monitoring of antidiabetic drug treatment during therapy
- Adherence to dietary recommendations
- Encouragement of multivitamin supplement intake
- Distribution of daily fat intake between three main meals
Overdose Effects
- Single doses of 800 mg and multiple doses of up to 400 mg three times a day studied without significant adverse findings
- Observation of the patient for 24 hours in case of significant overdose
