Description

  • Symptomatic treatment of mild to moderately severe Alzheimer's dementia
  • Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease

Dosage Instructions

  • Initial dose: Rivastigmine 1.5 mg twice a day
  • Dose titration: Rivastigmine 1.5 mg, 3 mg, 4.5 mg, and 6 mg twice a day based on tolerability
  • Maintenance dose: 3 mg to 6 mg twice a day
  • Re-initiation of therapy: 1.5 mg twice daily if interrupted for several days
  • Rivastigmine transdermal patch: Dose ranges from 4.6 mg/24 hours to 13.3 mg/24 hours based on tolerability and disease severity

Administration

  • Twice a day with morning and evening meals

Pharmacology

  • Binds reversibly with and inactivates cholinesterase
  • Prevents the hydrolysis of acetylcholine
  • Increases concentration of acetylcholine at cholinergic synapses

Interaction

  • Exaggerates effects of succinylcholine-type muscle relaxants during anesthesia
  • Should not be given concomitantly with cholinomimetic substances
  • No pharmacokinetic interaction observed with digoxin, warfarin, diazepam, or fluoxetine

Contraindications

  • Hypersensitivity to the active substance or other carbamate derivatives

Side Effects

  • Commonly reported gastrointestinal adverse reactions including nausea and vomiting
  • Higher susceptibility in female patients

Pregnancy & Lactation

  • Not recommended during pregnancy unless clearly necessary
  • Excretion into milk

Precautions & Warnings

  • Monitor body weight
  • Caution in patients with respiratory diseases, urinary obstruction, seizures, renal impairment, mild to moderate hepatic impairment, asthma, or obstructive pulmonary disease

Use in Special Populations

  • Increased exposure in moderate renal and mild to moderate hepatic impairment
  • Not recommended for use in children

Overdose Effects

  • Accidental overdose may result in symptoms such as nausea, vomiting, diarrhea, hypertension, hallucinations, bradycardia, and syncope
  • Conservative management recommended in most cases
  • Use of antiemetics and atropine in severe cases
