Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Rivastigmine is a carbamate derivative
- Structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase
- Prevents hydrolysis of acetylcholine
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase
Dosage
- Rivastigmine capsule initial dose: 1.5 mg twice a day
- Dose titration based on tolerability
- Recommended maintenance dose: 3 mg to 6 mg twice a day
- Rivastigmine transdermal patch initial dose: 4.6 mg/24 hours
- Dose titration after a minimum of 4 weeks
- Maximum dosage of transdermal patch: 13.3 mg/24 hours
Administration
- Administered twice a day with meals
Interaction
- May exaggerate the effects of succinylcholine-type muscle relaxants
- Caution with anaesthetic agents
- Should not be given concomitantly with other cholinomimetic substances
Contraindications
- Contraindicated in patients with hypersensitivity to carbamate derivatives
Side Effects
- Most commonly reported adverse reactions are gastrointestinal
- Nausea and vomiting are common, especially during titration
- Female patients more susceptible to gastrointestinal adverse reactions
Pregnancy & Lactation
- Not recommended during pregnancy
- Excreted into milk, should not be used during lactation
Precautions & Warnings
- Monitor body weight
- May exacerbate urinary obstruction and seizures
- Monitor for worsening of extrapyramidal symptoms
- Caution in patients with respiratory diseases, asthma, or obstructive pulmonary disease
- Not recommended for use in children
Use in Special Populations
- Increased exposure in moderate renal and mild to moderate hepatic impairment
- Dosing recommendations should be closely followed
- Not recommended for use in children
Overdose Effects
- Accidental overdose mostly asymptomatic
- Symptoms may include nausea, vomiting, diarrhea, hypertension, hallucinations
- Bradycardia and syncope may occur
- Conservative management recommended
