Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Carbamate derivative
- Cholinesterase inhibition
- Structurally related to physostigmine
- Binding with and inactivating cholinesterase
- Preventing hydrolysis of acetylcholine
- Increase in concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase
Dosage
- Rivastigmine capsule initial dose: 1.5 mg twice a day
- Dose titration: starting with 1.5 mg twice a day, increasing to 3 mg, 4.5 mg, and then 6 mg twice a day based on tolerability
- Effective maintenance dose: 3 mg to 6 mg twice a day
- Maximum daily dose: 6 mg twice a day
- Rivastigmine transdermal patch initial dose: 4.6 mg/24 hours
- Dose titration up to 13.3 mg/24 hours
- Administration twice a day with meals
Administration
- Twice a day with morning and evening meals
Interaction
- Exaggeration of succinylcholine-type muscle relaxants effects during anesthesia
- Caution with anaesthetic agents selection
- No concomitant use with other cholinomimetic substances
- Possible interference with anticholinergic medicinal products
- No observed pharmacokinetic interaction with digoxin, warfarin, diazepam or fluoxetine
Contraindications
- Hypersensitivity to the active substance or other carbamate derivatives
Side Effects
- Most commonly reported adverse reactions: gastrointestinal including nausea and vomiting, especially during titration
- Female patients more susceptible than male patients to gastrointestinal adverse reactions and weight loss
Pregnancy & Lactation
- No clinical data available
- Not recommended during pregnancy unless clearly necessary
- Excretion into milk in animals
- Not known if excreted into human milk
- Should not be used during breastfeeding
Precautions & Warnings
- Patients with sick sinus syndrome or conduction defects, respiratory diseases
- May increase gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitoring of body weight required
- Caution in asthma or obstructive pulmonary disease
- May worsen extrapyramidal symptoms
Use in Special Populations
- Dosing recommendations for renal and hepatic impairment
- Not recommended for use in children
Overdose Effects
- Accidental overdose cases mostly not associated with clinical signs or symptoms
- Possible symptoms: nausea, vomiting, diarrhea, hypertension, hallucinations, bradycardia, syncope
- Recommendations for management and use of antiemetics
- Use of atropine in massive overdose
- Scopolamine not recommended as an antidote
