Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Carbamate derivative
- Cholinesterase inhibitor
- Binds reversibly with and inactivates cholinesterase
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase compared with peripheral tissues
Dosage
- Initial dose: 1.5 mg twice a day
- Dose titration: 1.5 mg to 6 mg twice a day
- Maintenance dose: 3 mg to 6 mg twice a day
- Re-initiation of therapy: 1.5 mg twice daily
- Transdermal patch: 4.6 mg/24 hours to 13.3 mg/24 hours
Administration
Twice a day, with morning and evening meals
Interaction
- Exaggerates the effects of succinylcholine-type muscle relaxants
- Should not be given concomitantly with other cholinomimetic substances
- No pharmacokinetic interaction with digoxin, warfarin, diazepam, or fluoxetine observed
Contraindications
- Hypersensitivity to the active substance or other carbamate derivatives

- More susceptible in female patients
Pregnancy & Lactation
- Not recommended during pregnancy unless clearly necessary
- Excreted into milk
- Should not be used while breastfeeding
Precautions & Warnings
- Sick sinus syndrome or conduction defects
- Cholinergic stimulation may increase gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitor body weight
- Not recommended for use in children
Use in Special Populations
- Increased exposure in moderate renal and mild to moderate hepatic impairment
- Not recommended for use in children
Overdose Effects
- Most cases of accidental overdose have not been associated with clinical signs or symptoms
- Symptoms may include nausea, vomiting, diarrhea, hypertension, hallucinations, bradycardia, and syncope
- Conservative management recommended for asymptomatic overdose
- Use of antiemetics and atropine in severe cases
- Scopolamine as an antidote not recommended
