Indications
- Treatment of non insulin dependent diabetes melitus (NIDDM)
- Ineffective in completely pancreatectomized patients
- Requires insulin in juvenile-onset type of diabetes
- Oral therapy dangerous and doomed to failure for some patients
Pharmacology
- Orally effective hypoglycaemic agent
- Reduces blood sugar concentration
- Stimulates secretion of endogenous insulin
- Reduces basal hepatic glucose production
- Enhances peripheral insulin action at target sites
Dosage & Administration
- Initial stabilization dosage: Glibenclamide half tablet (2.5 mg) initially during or after breakfast
- Daily dose may be continued as maintenance therapy
- Increment of daily dose in steps up to a maximum of 3 tablets
- Change over from insulin to Glibenclamide
- Hospital supervision for changeover from insulin to Glibenclamide
Interaction
- Alcohol
- Cyclophosphamide
- Dicoumarol
- Monoamino oxidase inhibitors
- Phenylbutazone
- Propranolol and other beta-adrenergic blocking agents
- Certain long-acting sulphonamides enhance the hypoglycemic effect of Gluconil
Contraindications
- Severe metabolic de-compensation with acidosis
- Pre-comatose states and diabetic coma
- Pregnancy
- Diabetes mellitus complicated by fever, trauma or gangrene

- Nausea

- Epigastric pain
- Dizziness
- Weakness
- Paraesthesia
- Headache
- Allergic skin reactions
- Haemopoietic reactions (leukopenia, thrombocytopenia, etc.) occasionally observed
Pregnancy & Lactation
- No information on use in human pregnancy
- No apparent ill consequence in general use for many years
- No hazard in animal studies
- Not established whether transferred to human milk
- Other sulphonylureas found in milk
Precautions & Warnings
- Weight reduction is of the greatest importance in the treatment of diabetes
- Vigorous effort for weight reduction as part of diabetic treatment
Overdose Effects
- Hypoglycaemia symptoms
- Mild hypoglycaemic symptoms without loss of consciousness may be treated with oral glucose
