Ambrisan 5 mg (Tablet)

Indications

• Treatment of Pulmonary Arterial Hypertension (PAH) (WHO Group 1)
• Improvement of exercise ability
• Delay of clinical worsening

Pharmacology

• High-affinity ETA receptor antagonist
• High selectivity for ET₊ versus ET₋ receptor
• Peak concentrations occurring approximately 2 hours after oral administration
• Substrate of P-gp
• Highly bound to plasma proteins
• Elimination predominantly by non-renal pathways

Dosage & Administration

• Initial treatment of 5 mg once daily
• Can be increased to 10 mg once daily if 5 mg is tolerated
• Can be administered with or without food

Interaction

• Limit Ambrisentan dose to 5 mg once daily when co-administered with Cyclosporine

Contraindications

• Potential fetal harm
• Contraindicated in pregnant women
• Exclusion of pregnancy before treatment initiation
• Contraindicated in patients with Idiopathic Pulmonary Fibrosis (IPF)

Side Effects

• Decreases in hemoglobin concentration and hematocrit

Pregnancy & Lactation

• Pregnancy Category X
• Not recommended for breastfeeding

Precautions & Warnings

• Peripheral edema as a known class effect
• Consideration of pulmonary veno-occlusive disease
• Monitoring for hematological changes
• Not recommended in patients with moderate or severe hepatic impairment
