Indications
- Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent (Type II) diabetes mellitus (NIDDM) patients
- Use in combination with metformin for inadequate glycaemic control
- Use in combination with insulin for hyperglycaemia control
- Potential for hypoglycaemia with combined use of Adglim and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Determined by desired blood sugar level
- Initial dose: 1 mg once daily
- Gradual dose titration based on regular blood sugar monitoring
- Usual dose range: 1 to 4 mg daily
- Single daily dose timing based on lifestyle
Administration
- Required to be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, anabolic steroids, fluconazole, salicylates, sulfonamide antibiotics, tetracyclines
- Attenuating drugs: Calcium channel blockers, corticosteroids, diuretics, nicotinic acid
- Potential risk of masked hypoglycaemic symptoms with beta-blockers, clonidine, or reserpine
- Unpredictable effect with acute and chronic alcohol intake
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride or other sulfonylureas
- Severe hepatic dysfunction
- Severe impairment of renal function
- Not for dialysis patients

- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary for pregnant patients
- Harmful effect on breastfed child
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Careful monitoring and adjustment of Adglim dosage
- Prompt control of hypoglycaemia with immediate intake of carbohydrates
Overdose Effects
- Hypoglycaemia due to overdosage
- Treat mild symptoms with oral glucose and drug dosage adjustments
- Severe reactions require immediate hospitalization and glucose solution infusion
- Close monitoring for at least 24 to 48 hours
