Indications
- Adjunct to diet and exercise for lowering blood glucose in Type II diabetes mellitus
- Use in combination with metformin or insulin for glycaemic control
- Potential for increased risk of hypoglycaemia when combined with insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
Dosage
- Initial dose of 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range in well-controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses determined by physician
- Consideration for dose adjustment with changes in patient's weight or lifestyle

- Must be swallowed with sufficient liquid
Interaction
- Interactions with ACE inhibitors, anabolic steroids, fluconazole, MAO-inhibitors, and other drugs that potentiate hypoglycaemic action
- Interactions with barbiturates, corticosteroids, diuretics, and other drugs that may attenuate hypoglycaemic action
- Potential masking of hypoglycaemic symptoms with concomitant use of certain drugs
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for use in diabetic ketoacidosis or diabetic coma
- Contraindicated in patients hypersensitive to Glimepiride or sulfonylureas
- Not for use in severe hepatic dysfunction or severe impairment of renal function
- Contraindicated in dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Gastrointestinal symptoms like nausea, vomiting, and diarrhoea
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Contraindicated during pregnancy
- Changeover to insulin necessary during pregnancy
- Not to be taken during breastfeeding
- Changeover or discontinuation of breastfeeding necessary
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessitates careful monitoring and potential dosage adjustment
- Prompt control of hypoglycaemia with immediate intake of carbohydrates
Overdose Effects
- Potential for hypoglycaemia in case of overdose
- Treatment with oral glucose and adjustments in drug dosage or meal patterns for mild hypoglycaemic symptoms
- Hospitalization and continuous monitoring for severe hypoglycaemic reactions
- Rapid intravenous injection of concentrated glucose solution for hypoglycaemic coma
