Indications
- Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Use in combination with metformin and insulin for glycaemic control
- Potential for increased risk of hypoglycaemia with combined use of insulin
- Indicated in conditions where hyperglycaemia cannot be controlled by diet and exercise alone
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
Dosage
- Governed by desired blood sugar level
- Initial dose of 1 mg once daily, with gradual incremental increase based on monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician based on lifestyle
- Dose adjustment required with changes in insulin sensitivity and patient factors
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Drug interactions potentiating hypoglycaemic action (e.g., ACE inhibitors, aminosalicylic acid)
- Drugs attenuating hypoglycaemic action (e.g., calcium channel blockers, corticosteroids)
- Potential for masking warning symptoms of hypoglycaemic attack with certain concomitant treatments
Contraindications
- Not suitable for insulin dependent diabetes mellitus or diabetic ketoacidosis
- Hypersensitivity to Glimepiride or sulfonylureas
- Severe hepatic dysfunction, severe impairment of renal function, and dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhea, abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary for pregnant patients
- Harmful if taken during breastfeeding
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Prompt control of hypoglycaemia with immediate intake of carbohydrates
Overdose Effects
- Potential for producing hypoglycaemia
- Treatment with oral glucose and adjustments in drug dosage or meal patterns
- Severe cases may require rapid intravenous injection of concentrated glucose solution and continuous infusion
